Relationship between CD_ (34) antigen expression and high P-gp expression and prognosis of acute leukemia

Abstract Objective To explore the relationship between acute leukemia (ADCD34 antigen expression and high P-gp expression and prognosis. Methods Simultaneous high expression of anti-CD34 and anti-P gp on mononuclear cells of 30 AL patients can be used as an indicator of poor AL prognosis.

P―gp acute leukemia prognosis for future inspection.

1.2.2 Indirect immunofluorescence labeling: Wash 1X106 / ml cells twice with PBS solution, discard the supernatant, add 1:10 diluted anti-Pg monoclonal antibody UIC2 (or anti-CD34 monoclonal antibody) 10 (M, Incubate in a 37 * C water bath for 30 minutes, wash with PBS, discard the supernatant, add 1: 200 diluted goat anti-mouse fluorescein isothiocyanate (FITC-IgG) to centrifuge and wash excess antibody, and add PBS 1.0ml before using the machine. Filter through a 500 mesh screen.

1.2.3 Flow cytometry detection method: using FACS420 type flow cytometer, laser light source 2W hydrogen ion laser. Output power computer.

Result judgment: P-gp expression rate> 2% is high expression (+), 30% is positive expression (+), 1.3 treatment regimen and efficacy judgment 1.3.1 Induction chemotherapy regimen for newly-treated patients: ANLL to DA (flexible Erythromycin + cytarabine) or MA (mitoxantrone + cytarabine) 3 + 7 is the main program, and ALL is mainly based on the 28-day program of VDLP.

The high expression of the coding product P-glycoprotein (P-gp) is the cause of tumor cell resistance and poor prognosis. This article analyzes the expression of P-gp and CD34 antigen in AL patients hospitalized from January 1999 to January 2002, and explores the relationship between it and clinical efficacy. The reports are as follows.

1 Materials and methods 1.1 Clinical data 30 newly diagnosed AL patients, according to FAB diagnostic criteria, 16 cases of acute non-lymphocytic leukemia (ANLL) (M, 2 cases, M2a6 cases, M4a2 cases, M4c2 cases, M5a1 cases, M5b1 cases), There were 13 cases of acute lymphoblastic leukemia (ALL) (5 cases of Li, 26 cases of L26, and 32 cases of L32), and 1 case of acute mixed cell leukemia (MAL). There were 14 males and 16 females, with a median age of 38 years (18-78 years). Most of them were confirmed by immunological typing and cytogenetics. All cases were tested for CD34 and P-gp simultaneously.

1.2 Detection method 1.2.1 Specimen: Take 2ml of heparin anticoagulated bone marrow fluid and slowly inject it into the upper layer of the lymphocyte separation solution. The density gradient is separated for 20 minutes (2000r / min), draw the upper layer of mononuclear cells, wash 3 times with RPMI1640, 1.3. 2 Efficacy determination: good efficacy standard: â‘  1 course of treatment achieves complete remission (CR); â‘¡ 1 course of treatment does not relieve but the peripheral blood leukocytes have the lowest myeloid leukemia cell decline index (MBDI)> 60%. Poor efficacy standard: does not meet the above index.

2 Results Table 1 gp expression group in AL 2.2 CD54 and P * gp expression and clinical efficacy (see Table 2).

The relationship between gp expression and clinical efficacy The number of cases with good curative effect and poor curative effect The total clinical curative effect in the 4 groups is from small to large. The clinical curative effect of patients with P * gp + / CD34 is the best. The difference between the two is significant. Significant significance (p 3 discusses P-gp is a membrane protein encoded by the multidrug resistance gene MDR1, which can pump intracellular chemotherapeutic drugs out of the cell and produce drug resistance, resulting in a poor clinical prognosis. A retrospective analysis of 30 AL patients with CD34 and P-gp found that their expression has a certain relationship with clinical efficacy: â‘  The expression of CD34 and P-gp in ALL and ANLL, the difference was not significant 0.05); â‘¡ Of the 13 cases with high expression of P-gp, only 3 cases had good clinical efficacy (3%) and 15 cases with CD34 positive expression had good efficacy in 6 cases (40%). The difference between the two was significant (p
